"Cost of treatment does not include: ultrasound monitoring, blood work, medications, embryo cryopreservation, storage, thaw and transfer." Two and three cycle plans ($10,000, $12,000) state they include ICSI and assisted hatching; PGT-A and anesthesia are not mentioned for the single cycle plan.
